So what do you need?
The Financial evidence
Either 800,000 THB (or equivalent) in your bank account at your country OR 65,000 THB or equivalent in consistent deposits for the past 12 months.
The Paperwork
A Police Report either from your local state or province or federal government. Click this link to learn how to get it. The same link will give you a form you take to your doctor or hospital to sign as well.
Insurance
Ask your current insurance company if their policy covers you when you travel. They will issue you a coverage letter which you will use in your application.
The rest is just copies of passport, photo, etc.
