You need a completed TM.47 form, copies of key passport pages, your TM.6 card, any previous TM.47 slip, and – if mailing – a self-addressed stamped envelope.
Double-check that every passport copy is clear and includes the full page. Blurry scans or missing margins cause rejections. Keep the original previous TM.47 slip in a safe place; immigration often asks to see it.
